you are a natural or legal person acting in a business capacity holding a licence to handle addictive substances and preparations and you no longer wish to handle these substances or you no longer actually handle them.

What you need if you are using this service

Fill in the application form which you will find on the Ministry of Health's website and return the hardcopy of the relevant permit/license.

Where and how to solve this service

You can send the form by post using a registered letter with acknowledgement of receipt, or submit it in person, subject to proper identification at the Ministry’s mailroom at the following address:
